    Medical Release

    I agree that I am the owner of the above dog(s) and give permission to On The Ball K9 Training LLC to act on my behalf, and in my pet(s) best interest, by providing veterinary care at my expense for illness or injury. In the event any of my animals become ill, On The Ball K9 Training LLC is authorized to take them to either my veterinarian or selected after-hours emergency clinic to diagnose their condition. The veterinarian is to call me for authorization to treat. If I am unavailable and this is an emergency, the veterinarian is hereby authorized to treat the animal at their discretion. I understand that On The Ball K9 Training LLC cannot be held responsible for the treatment provided by the veterinary clinic or the loss of my pet(s).The charges for any vet visit or treatment will be my responsibility with the veterinarian or clinic.
    I understand because of the inherent nature of dogs, the behavior of canines is unpredictable and it is not unexpected that bites or other injuries could occur to my pet from another dog, or by other unavoidable causes. On The Ball K9 Training LLC agrees to provide reasonable care to prevent injury or illness to my pet(s). However, in the event of injury or illness, the owner of On The Ball K9 Training LLC shall not be held personally liable for such injury or illness.
